Upper School Science Fair

On March 11th, the NCCS Upper School will present a Science Fair.  All Upper School students will be creating and presenting.  Come and check it out, Thursday, March 11th, 7:30-8:30 pm, at the Upper School.

Elective Registration Form

The Elective Registration Form is used each school year for selecting specific before and after-school electives of interest.  Each elective has their own cost, time, and requirements.  This form will be updated as elective offerings change.

Please print the form out and use it to:

1. Select and communicate each of your children’s desired electives (one form per child, please).
2. Determine the total amount due to NCCS for the selected electives.

Field Trip Permission Form

The Field Trip Permission Form is used for any school-sponsored, student activity that pertains to the classroom or coursework.  Athletic-based trips are covered by a separate process.  When directed by your child’s teacher, print out the form via the link below and complete all appropriate information.

Check Reimbursement Form

Use the Check Reimbursement Form for requesting reimbursement from the school for PRE-APPROVED expenditures.  Only those expenditures that have been approved by the school’s review and approval process will be paid.  Expenditures not pre-approved will be the responsibility of the person making the expenditure.

ECP Description and Prepaid Credits Explanation Form

This form contains a description of the Extended Care Program, its charges, and an explanation of the Prepaid Credits Program.

ECP Prepaid Credits Form

Use this form to pay for ECP Prepaid Credits.  The form contains a description of the process, a space to indicate the children being covered by the credits, and space to indicate medical issues, hobbies, and other information about each child.  Print this form, fill it out, sign and date it, and return it and a check made out to NCCS to the school office.
